Our website contact form uses Web3Forms to receive and send enquiries to AJQ Care Advisory. When you submit the form, the details you enter are processed by Web3Forms and forwarded to us. Web3Forms may also process technical information associated with the submission for delivery, security and spam prevention.
Please use the form for an initial enquiry. Do not include medical records, account numbers, financial documents or detailed health information in the form. We can discuss an appropriate way to provide further information if it is needed.

Why we use your information
We use personal information to:

We do not sell personal information.
Overseas processing
Web3Forms operates from India and uses infrastructure and service providers that may process contact form information in other countries, including through services in Europe and the United States. The exact locations may depend on its infrastructure arrangements. Other technology providers used by AJQ Care Advisory may also store or access information overseas.
Keeping information secure
We take reasonable steps to protect personal information from loss, misuse and unauthorised access or disclosure. These steps include password-protected accounts, secure storage with restricted access, and only sharing information when it’s authorised or required by law.
No method of sending information online is completely risk-free. For that reason, we ask you to keep detailed health and financial information out of the initial website contact form.
How long we keep information
We keep information for as long as reasonably needed to respond to enquiries, provide services, maintain appropriate business records and meet legal obligations. When information is no longer needed, we take reasonable steps to delete or de-identify it.
Web3Forms applies its own retention arrangements to contact form submissions. Its published policy states that it retains submission data for a maximum of three years, unless a shorter period applies or it is deleted earlier.
